The screenshot is just the one from code2, the icon pack isn't released by him yet. I just had no time to use it, and it's- as I said- only the dekorator theme by Yaccin modified so it REALLY looks like Kore for Emerald.
You can find pretty much everything you asked for exc the icons in code2's KORE suite.

Can you detail the name of your icon pack, of your window decoration and what the soft you're using instead of the K launch menu ?
