That's KDE 4.0 original Oxygen theme updated to KDE 4.5 functionality, including Oxygen Lancelot theme.
The only difference is pager, which uses new blue skin, I like it more than old white one.
But if anyone wants pure 4.0, old pager is in main BlackOxygen dir. Just copy svg to the "widgets" directory.
Installation instructions:

Copy BlackOxygen dir to desktop theme loction (usually /usr/share/kde4/apps/desktoptheme)
In case of no root access or You might wish to install theme in your home directory, put the BlackOxygen dir in ~/.kde/share/apps/desktoptheme
Now BlackOxygen should be availble on the list of Plasma themes

Known issues:

System tray incorrectly shows underline in the middle of the systray icons. It happen only when you apply the theme and panel is not wide enough to display icons underlined. Change of panel width fixes the issue, after that original size can be restored and systray is displayed correctly.

actionOverlays.svgz is missing the arrowup icons
Thanks for the tip, fixed in 1.0.1 Please download and retest. BTW it seems that Oxygen theme has this third icon missing from the action-overlay set too.
Yeah, oxygen is missing that too. Unfortunately is not a surprise that some kde features aren't updated properly between releases. Thanks for the fix!
